New Patient Paperwork

WHAT TO EXPECT

When you call our office for your first appointment, one of our friendly staff will schedule a time that’s convenient for you. We will request that you complete our patient forms located in the New Patient Center. This paperwork provides us with your health history and information on your condition.

When you meet with your doctor, you’ll be able to talk about your specific concerns and questions. Your doctor may ask you about your health condition and lifestyle to gain a more complete picture of your treatment needs.

If your condition(s) can be helped with chiropractic treatment, you will have a comprehensive examination that includes orthopedic, neurological, and postural testing. Based on the findings of these tests, x-rays or other types of diagnostic imaging may be needed to help us give you the best care possible.

After reviewing your health history, goals, examination, and x-rays, your doctor will then discuss your specific health condition with you and then recommend a care plan designed to address your unique health problem. Your treatment may include spinal or joint adjustments, and/or corrective exercises. You will also be given specific instructions regarding activities to do to – or to avoid – at home.
